Before your appointment:
If any personal information has changed, or you are a new patient, please complete a new Patient Information Form and Medical History. Make sure to click “Submit”.

When you arrive for your appointment:
Stay in your car and call (319) 472-3282. Over the phone we will review COVID-19 screening questions. Please bring a mask with you and wear it in our office. If you do not have a mask, one will be provided for you. Only patients and their guardian will be allowed into the office. Any additional visitors will be asked to wait in the car.

If you experience COVID-19 symptoms within 14 days of your appointment, please let us know.
